If my ex-wife refused to sign my refinance papers to have her name taken off the house and get my new wife’s name on, can I get her for a show cause?
My decree says she has no right to the house and she has to cooperate with the sale of the house.

I am sorry for your situation. One would need to read the decree and the paperwork associated with it in its entirety to give you a solid answer here. But if as you say she must cooperate with a transfer - this is technically not a "sale" - you need to take her back to court to enforce your agreement or decree by asking a judge to hold her either in breach or contempt of court.
