My child has a heart condition and can work. However my ex is taking me to court to extend child support until my child is 21.

Consult with a family or divorce law attorney--there is potentially too much at stake to not have an attorney represent you. The short answer is, I do not believe that you can be forced to support an adult child, even one who is disabled, unless there had previously been a child support agreement or order to that effect (i.e. coming out of the divorce); however, since every case is different, it is vital to consult with an attorney.

Laws may vary from state to state, and sometimes change. Tiny variations in the facts, or a fact not set forth in a question, often can change a legal outcome or an attorney's conclusion. Unlike the information in the Answer(s) above, upon which you should NOT rely, for personal advice you can rely upon we suggest you retain an attorney to represent you.
