If I’m a single mother, how can I move out of state with my children?
The father and I have never been to court to establish custody. He doesn’t make an effort to be in the children’s lives. I would like to move out of state in a couple months and I would like to know what, if anything, needs to be done so that I can do so?

Unless there is a custodial order in place where the "ex" has custody of your children you have the legal right to move out of state with them with or without the "ex's" knowledge. Before you do so, I suggest that you first consult with a family law attorney as to your intent.
