If I gave my roommate a 30 day notice 2 1/2 months ago but agreed she could stay for a little extra time while she looked for a place, is the notice now void because I let her stay longer?
Even though in the 30 day notice it stated her month-to-month lease would not be extended past the end on the month in which I gave it?

Yes, giving your roommate an extension of time to stay voids the notice you had given her. You need to provide her with new notice in order to evict her, unless she has given you some other good grounds to evict (e.g. not paid rent; breached material lease terms; etc.).
