What to do if I had out patient surgery over 3 years ago and the hospital neglected to bill my insurer in a timely manner?
Thus the insurance company is unwilling to pay the amount due. Unbeknownst to me the hospital bill us for it and have since been put on collections. The amount isn’t much, however I was wondering if we have any recourse?

Your recourse would be to make a complaint with your state's department of insurance due to the failure of your carrier to pay for the surgery. Another option would be to file suit against your carrier for the amount, possibly in small claims court.
