You can file an amendment to your bankruptcy to include the new debt and name the creditor. The bankruptcy court has the form you will need to file an amendment. There is a filing fee for filing the amendment. The bankruptcy court clerk can tell you what the current filing fee is for filing an amendment to your bankruptcy.
