If my ex-wife was required to carry health insurance for our 2 kids as part of our divorce decree but she has not done so, what am I legally required to do?

What am I legally required to pay for in regards to medical care?

You should cover your children in order to mitigate your damages and go to court to seek a contempt hearing and file a motion for reimbursement of monies you need to pay to have them covered. That would pretty much be the most direct way to ensure you get the coverage for them and get the monies owed so you are not expending extra.
