The questions on forms when filing a claim, there is a lot of stuff that has nothing to do with the claim. Just answer what appears to be relevant to your particular claim.  The forms are likely for all types of claims, so not all of it will be appropriate to an auto theft claim.  Regarding the prior insurance company, they just want to make sure you didn't report a theft to them, as well.  They are trying to look for fraudulent claims.  If you leave out a pertinent answer, they will certainly let you know when they get your form.
