What to do if my ex-wife piereced my 12 year old daughter’s nose and my 10 year old son’s ear against my wishes?

In the papers it says we have to make all decisions together. Is this considered contempt of court and get i get full custody with this type of action. She did the piercing by herself not even at a licensed facility.

I suggest that you immediately consult with a family law attorney about the perceived violation of a court order and perhaps "child abuse" by your former spouse with respect to the piercings that you have written about.
