My insurance company paid for my back surgery, not the workmans comp. provider. Are they entitled to monies from my workmans comp. settlement?

Typically WC pays your medical bills directly, but if your workers' comp settlement includes the money to pay your medical bills, (in addition to covering your other expenses), then some health insurers "subrogate", i.e., legally request their money back, from the settlement, and they may legally do so.
