What to do if my wife wants us to give our secondary vehicle to her sister?

The vehicle needs repair. If she has an accident (even if all other paperwork is properly completed), can I be held liable for not having vehicle repaired previously?

You need to have the vehicle inspected and then transfer registration to her or if you transfer make sure the inspection is done immediately so that registration is finalized with the inspection by the sister. If there is significant issue with this car (not road worthy), inspection won't pass and she won't be given a registration for said vehicle. Sign over the car to the sister and go with the sister to get it inspected and registered; this is the safest and cleanest way.
