My relative is creating a new power of attorney and is not sure if she has previous
poas or who they would have be assigned to or sent to. Is there a way to revoke all
previous poas in one document. She lives in Connecticut if that makes a difference

Yes; this can be done very simply. The new power of attorney needs to simply state that it "Supercedes and revokes all prior powers of attorney created by me, whenever created and regardless of who was therein made my agent or attorney in fact."
