What paperwork do I file with the court to get my child removed out of an unsafe enviroment with her father?

No custody ever been set up; he is refusing to give her back from visitation.

If the child is in danger, you need to report the dangerous situation to your local law enforcement or children's protective agency. 

Your next focus needs to be on filing the appropriate paperwork with the court to get custody of your child so that you have an order in place.  Without a formal order, kid snatching will continue to be a problem with few remedies.  If you are married, the custody paperwork can be filed through a divorce proceeding.  If you have not been married, then you'll file for a basic custody suit.
